Financial Policy!

1. Payment Responsibility:   

Payment for all treatment is due at the time of service unless prior arrangements have been made. Patients (or the patient’s legal guardian) are responsible for all fees, regardless of insurance coverage. Our Office accepts cash, personal checks, credit and debit cards. There is a $50 fee for returned checks. Balances over 90days are subject to additional collection fees and interest charges of 3% per month.

2. Dental Insurance:     

Our office is Not in-network with any insurance carriers. As a courtesy, we will help you process all your dental insurance claims*. Please be aware that it is not possible for us to provide an amount that will be covered by your insurance carrier for a particular procedure.           

*Insurance coverage is subject to limitations, exclusions, waiting periods, frequency, age restrictions, deductibles and maximum allowances which can affect the amount paid for procedures. Any insurance payment will be directly sent to you. Our office does not accept any payment from insurance carriers. All charges that you incur in our office are your responsibility regardless of your insurance coverage. We will assist you in any way we can to ensure that you receive the optimum benefit available under your particular plan. 

3. Broken or Missed Appointments:    

In order to provide the best services to our patients, we require 24 hour notice for any change or cancellation. A $50 charge will be assessed for no shows, short notice cancellation. Emergencies, illnesses, etc are taken into consideration.
